This article provides a brief overview of various definitions and activities of public health, normative concepts and principles integral to public health, and an introduction to several substantive ethical questions and tensions central to public health ethics. The core of public health ethics concerns the moral justification of policies, programs, and the law to protect and promote public health. Because public health encompasses broad social programs and public policies, such as the provision of clean fluoridated water and bans on public smoking, questions regarding the justification of paternalistic interventions, fair distribution of health, and responsibility for health are prominent. Discrete patient care interventions e.

At its core, public health is concerned with promoting and protecting the health of populations, [ 1 ] broadly understood. Promoting and protecting the health of populations often involves or requires coordinated action on the part of governments. All nations and most states or provinces and municipalities have health departments whose functions include everything from the inspection of commercial food service to the collection and use of epidemiological data for population surveillance of disease. Collective action to promote and protect population health also occurs at the global level, as exemplified by the activities of the World Health Organization. This entry provides a general lay of the land of the central ethical issues and challenges that arise in public health policies and practices and that drive the literature in public health ethics. Some issues are conceptual: what is the public, and in what sense can a public be healthy? Other questions concern the moral foundations of the enterprise: what sort of value does public health have, and what morally justifies efforts to promote public health?

Recently we argued that social justice is concerned with human well-being, which is best understood as involving plural, irreducible dimensions, each of which represents something of independent moral significance. Health is one of these distinct dimensions of well-being, as is personal security, the development and exercise of cognitive capacities for reasoning, living under conditions of social respect, developing and sustaining deep personal attachments, and being able to lead self-determining lives. In this paper, we address why considerations of justice, and not utilitarian aims as applied narrowly to health outcomes, are most foundational to public health. In particular, we argue that the aspiration for improvement of the health of populations defines the positive aim of justice in public health, along with the negative aim of reducing or combating systematic disadvantage that affects adversely historically situated social groups and, more generally, children across the normal life span when their well-being is not assigned a special priority in the development of public health policies. This course explores the main answers that have been given to this question in the modern West. We start with a survey of the major political theories of the Enlightenment: Utilitarianism, Marxism, and the social contract tradition. In each case, we begin with a look at classical formulations, locating them in historical context, but then shift to the contemporary debates as they relate to politics today.
