We who have committed our careers to promoting public health need to change our framework to encompass war as 1 of the most significant threats to the health of people in every demographic group and in every country. Practitioners, educators, and other workers in public health can play powerful roles in preventing war itself, as well as mitigating the public health consequences of war.

War is an intense armed conflict [a] between states , governments , societies , or paramilitary groups such as mercenaries , insurgents , and militias. It is generally characterized by extreme violence , aggression , destruction, and mortality, using regular or irregular military forces. Warfare refers to the common activities and characteristics of types of war, or of wars in general. While some scholars see war as a universal and ancestral aspect of human nature , [3] others argue it is a result of specific socio-cultural, economic or ecological circumstances. The word is related to the Old Saxon werran , Old High German werran , and the German verwirren , meaning "to confuse", "to perplex", and "to bring into confusion". The earliest evidence of prehistoric warfare is a Mesolithic cemetery in Jebel Sahaba , which has been determined to be approximately 14, years old. About forty-five percent of the skeletons there displayed signs of violent death.

The democratic peace theory posits that democracies are hesitant to engage in armed conflict with other identified democracies. Those who dispute this theory often do so on grounds that it conflates correlation with causation , and that the academic definitions of 'democracy' and 'war' can be manipulated so as to manufacture an artificial trend. Though the democratic peace theory was not rigorously or scientifically studied until the s, the basic principles of the concept had been argued as early as the s in the works of philosopher Immanuel Kant and political theorist Thomas Paine. Kant foreshadowed the theory in his essay Perpetual Peace: A Philosophical Sketch written in , although he thought that a world with only constitutional republics was only one of several necessary conditions for a perpetual peace. Kant's theory was that a majority of the people would never vote to go to war, unless in self-defense. Therefore, if all nations were republics, it would end war, because there would be no aggressors. In earlier but less cited works, Thomas Paine made similar or stronger claims about the peaceful nature of republics.
